The FD-1164 is the Identity History Summary Request card. The FD-258 is an applicant card generally used when an employer or agency requests a different US background check.
How to get your FBI background check from France

Start the FBI request
Submit your Identity History Summary request through the FBI and keep the email containing its QR code.

Book the fingerprint appointment
Choose an available appointment at 26 place du Marché Saint-Honoré, 75001 Paris. It is €150 TTC per person and takes about 45 minutes.

Bring the two essentials
Bring your valid passport and the FBI email containing your QR code. Sequana supplies the fingerprint cards.

Have both cards completed
We take the rolled and flat impressions, complete the identifying fields with you, and check each impression before you leave.

Submit by the route you chose
You receive two completed FD-1164 cards. Follow the current FBI or approved Channeler instructions for the route attached to your request.
